Infection in Primary Hip and Knee Arthroplasty

چکیده
Since the advent of prosthetic joint replacement, patients suffering from bone and joint pathology have benefited from significant improvements in mobility and pain relief. In Australia 39,200 hip replacements and 39,500 knee replacements were performed in 2009 (Australian Orthopaedic Association National Joint Replacement Registry [AOA NJRR]) (Graves, et al. 2010). With an ageing population, the number of patients undergoing these procedures is projected to increase significantly over time. Data from the United States of America predicts that by 2030, the number of patients undergoing primary hip and knee replacement will increase by 174% and 673% respectively (Kurtz, et al. 2007). The major complication of such techniques is infection of the prosthetic device, which is associated with significant costs to individual patients and to the public health system. Significant morbidity is associated with prosthetic joint infections including the need for further operative procedures, long-term antibiotic therapy, and prolonged hospitalisation. Thereafter, the mortality rate from prosthetic joint infection is estimated to be between 1.0 to 2.7 precent (Ahnfelt, et al. 1990, Zimmerli 2006, Zimmerli, et al. 2004). Aside from the effects on the individual patient, the financial cost to the health system is considerable. The estimated hospital costs is $ 96 166 (US) per patient requiring revision arthroplasty for infection, which is 4.8 times the cost of a primary arthroplasty(Bozic & Ries 2005). This chapter examines the underlying epidemiology, diagnosis, treatment and challenges in managing this problem.

Partial 2-Stage Revision in Chronic Hip Arthroplasty Infections: A Review
Background: Periprosthetic hip infections (PHIs) are troublesome complications of hip arthroplasties. The gold standard procedure for treating chronic PHI is a 2-stage approach. Recently, however, more conservative approaches have been developed to spare the osseointegrated components and avoid sequestra, bone loss, devascularization, and difficult reconstructions. The partial ...
متن کاملStaphylococcus aureus Colonization in Patients Undergoing Total Hip or Knee Arthroplasty and Costeffectiveness of Decolonization Programme
Background: Periprosthetic joint infection is a devastating complication of total joint arthroplasty. It seems that thepatient’s skin, nose, throat, and urine are important sites for microbial colonization. Colonization with staphylococcusaureus, especially methicillin resistant increases the risk of periprosthetic joint infection. The aim of this study was toassess the prevalence of staphyloco...
متن کاملAntibiotic Prophylaxis and DAIR Treatment in Primary Total Hip and Knee Arthroplasty, A National Survey in The Netherlands
BACKGROUND To prevent postoperative infection the use of systemic antibiotic prophylaxis is common ground. Type of antibiotic used and duration of prophylaxis are subject to debate. In case of suspected early periprosthetic infection a debridement, antibiotics and implant retention (DAIR) procedure is treatment of first choice. This study evaluated the antibiotic prophylaxis and DAIR treatment ...
متن کاملCharacterization of hip and knee arthroplasties and factors associated with infection☆
OBJECTIVE To characterize arthroplasty procedures, calculate the surgical infection rate and identify related risk factors. METHODS This was a retrospective cohort study. Data on operations performed between 2010 and 2012 were gathered from documental sources and were analyzed with the aid of statistical software, using Fisher's exact test, Student's t test and the nonparametric Mann-Whitney ...

Objective: The current study aimed to characterize the variation of serum PCT and IL-6 levels and look the specificity and the sensitivity in diagnosing pneumonia, urinary and superficial surgical site infections during the perioperative period of primary hip and knee arthroplasty. Methods: This observational study included 73 patients undergoing primary hip and knee arthroplasty. Serum Procalc...
متن کاملDesign, Implementation, and Applicability Evaluation of Hip and Knee Arthroplasty Registry
Introduction: Arthroplasty is a major orthopedic operation with an increasing rate. The success of this operation can significantly reduce patients’ pain and disabilities. This study aimed to design a registry system for hip and knee arthroplasties. Method: A comprehensive search was conducted to retrieve minimum data set from articles, guidelines, forms and reports published by orthopedic soci...
